Privacy Policy
for Use of Website, Social Media and Any Other Electronic Communications

Welcome to our Website!  We’d like it to be as useful as possible, but medical practice websites should be viewed differently than personal or other business Websites.  Therefore, for your own safety and privacy -- and to try to ensure respect for all persons -- we have the following Privacy Policy.  Additional terms relevant to Privacy may be found elsewhere in this Policy, in the policy posted on our Website named “Terms of Use and Privacy Policy,” and elsewhere on our Website.

Our Websites are not private or secure.  Please be aware most information transmitted over the Internet or by email is not secure; thus, confidentiality cannot be guaranteed.

Third-Party Sites

If we provide links to other websites, you are subject to that website’s privacy policy once you follow the link to the third-party website.  The Practice has no control over such third-party sites.

Posting and Security

We advise you not to post anything of a sensitive or medical nature on any website or social media site or by email.  You expressly acknowledge and agree that you assume all responsibility related to the confidentiality and security risks inherent in sending any content over, or posting anything on, the Internet and/or by email.  If you are a patient of our Practice, please be aware that even a seemingly innocuous or simple post to this page may reveal that you are a patient of our Practice.

PLEASE ALSO BE AWARE THAT INFORMATION TRANSMITTED OVER THE INTERNET or by email CANNOT BE 100% SECURE; THUS, CONFIDENTIALITY CANNOT BE GUARANTEED.  BY ITS VERY NATURE, A COMPUTER SYSTEM OR WEBSITE CANNOT BE ABSOLUTELY PROTECTED AGAINST EVERY INTENTIONAL OR MALICIOUS INTRUSION ATTEMPT.  FURTHERMORE, THE PRACTICE DOES NOT CONTROL THE DEVICES OR COMPUTERS OR THE INTERNET OVER WHICH YOU MAY CHOOSE TO SEND INFORMATION AND THEREFORE CANNOT PREVENT SUCH INTERCEPTIONS OR COMPROMISES TO YOUR INFORMATION WHILE IN TRANSIT TO THE PRACTICE.  THEREFORE, THE PRACTICE HEREBY MAKES NO GUARANTEE AS TO SECURITY, INTEGRITY OR CONFIDENTIALITY OF ANY INFORMATION TRANSMITTED TO OR FROM THIS WEBSITE OR STORED WITHIN THIS WEBSITE, OR THE PRACTICE.  BEYOND OUR REASONABLE CARE TO SAFEGUARD YOUR INFORMATION WHILE UNDER OUR CONTROL, THE PRACTICE CANNOT AND DOES NOT GUARANTEE THE ABSOLUTE SECURITY OF ELECTRONIC COMMUNICATIONS OR TRANSMISSIONS BECAUSE ANY TRANSMISSION MADE OVER THE INTERNET or by email BY ANY ORGANIZATION OR ANY INDIVIDUAL RUNS THE RISK OF INTERCEPTION.   IN ADDITION, WE HEREBY MAKE NO GUARANTEE AS TO SECURITY, INTEGRITY OR CONFIDENTIALITY OF ANY INFORMATION TRANSMITTED TO OR FROM THIS WEBSITE, OR STORED WITHIN THIS WEBSITE OR THE PRACTICE.  LIMITATION OF LIABILITY:  YOU ASSUME THE SOLE RISK OF TRANSMITTING YOUR INFORMATION AS IT RELATES TO THE USE OF THIS WEBSITE, AND FOR ANY DATA CORRUPTIONS, INTENTIONAL INTERCEPTIONS, INTRUSIONS OR UNAUTHORIZED ACCESS TO INFORMATION, OR OF ANY DELAYS, INTERRUPTIONS TO OR FAILURES PREVENTING THE USE OF THIS WEBSITE.  IN NO EVENT SHALL THE PRACTICE BE LIABLE FOR ANY DIRECT, INDIRECT, SPECIAL, CONSEQUENTIAL OR MONETARY DAMAGES, INCLUDING FEES, AND PENALTIES IN CONNECTION WITH YOUR USE OF OUR WEBSITES AND/OR ANY CONNECTIVITY TO OR FROM OUR WEBSITE.

Cookies, Tracking, and Website Statistics

We respect your privacy.  We do use Google Analytics to track and measure how users interact with our website’s content.  However, we understand (at the time of this writing in December 2015) that although Google Analytics uses cookies, Google Analytics does not store any personal information about our website users.  However, because we do not collect protected (personal) health information on our own website, Google Analytics does not even see your protected (personal) health information (PHI).  You may learn more about Google Analytics (including not limited to any changes Google may implement after this writing) at https://developers.google.com/analytics/devguides/collection/analyticsjs/cookie-usage?hl=en or Google’s other and possible future webpage(s) discussing this service.

The Information We Collect

We collect information from patients and prospective patients electronically through Instant Medical History (“IMH”) and its website as described below.  If you plan to use our website to link to IMH’s website to provide medical history, medical information and/or protected health information (“PHI”), please read the separate notices (on 2 Webpages) which you will be presented prior to your use entitled “Why we Use Instant Medical History” and “Important Information about your use of Instant Medical History.”  Such information is not shared with any third-parties unless authorized by law in association with patient treatment in accordance with all health related regulations, or when ordered by a court to do so, or with your specific permission.  We also collect information to learn how users interact with our website’s content, as discussed in the section above, “Cookies, Tracking, and Website Statistics”. We do not collect your or anyone’s protected health information on our own website, so Google Analytics does not even see your personal or protected health information (PHI).
