At Gail C. S. Anderson, M.D., P.A. (the “Practice”) we understand and respect your privacy and the concerns you may have in regard to privacy and data security. We have taken reasonable steps to protect the privacy of information our patients and potential patients provide to us. The following information outlines how the Practice treats information provided to it electronically.
As you may recall from the prior webpage, we believe Instant Medical History (“IMH”) will help improve the quality of care we can give you. At the end of this page, you will find a link to the IMH website so that you may provide your health information to us via IMH.
When you use IMH, the system establishes a secure communication link using Secure Sockets Layer (SSL) cryptographic protocols to provide communications security over the Internet. SSL technology encrypts segments of the network connections above the transport layer, using symmetric cryptography for privacy and a keyed message authentication code for message reliability.
After you complete the IMH history questionnaire and click done, by that click you will send your information to IMH’s computers (and/or those of its parent company, Primetime Medical Software) where the information is encrypted using a proprietary encryption algorithm with 128-bit technology to encrypt the file with an additional encryption layer using an internal program key. A secondary system helps protect messages from being decrypted by anything other than the software at the Practice. While data in transit over the Internet may be subject to sniffing and other data-theft technologies, with the high level of encryption supplied by these software programs, a stolen file is practically unreadable/useless and extremely difficult to decrypt. The software sends the encrypted file to the IMH Secure Server where it is held in a secure environment until the Practice’s computer retrieves the file over the Internet for local decryption (the file is not decrypted until it reaches our destination). IMH Secure Server is housed in a secure facility. The Practice’s computer that acts as its server is stored in a secured location. The practice also utilizes additional security measures including hardware firewall, security software and password protection to access computers containing Protected (personal) Health Information (“PHI”).
By using Instant Medical History (“IMH”), and the associated websites and/or computer systems to provide to the Practice your personal information, you accept the terms below. Please review this policy with each use as it may change without notice.
EVEN WITH THE ABOVE MEASURES, PLEASE BE AWARE THAT INFORMATION TRANSMITTED OVER THE INTERNET CANNOT BE 100% SECURE; THUS, CONFIDENTIALITY CANNOT BE GUARANTEED. ALSO PLEASE NOTE THAT, BY ITS VERY NATURE, A COMPUTER SYSTEM CANNOT BE ABSOLUTELY PROTECTED AGAINST EVERY INTENTIONAL OR MALICIOUS INTRUSION ATTEMPT. FURTHERMORE, THE PRACTICE DOES NOT CONTROL THE DEVICES OR COMPUTERS OR THE INTERNET OVER WHICH YOU MAY CHOOSE TO SEND CONFIDENTIAL PERSONAL INFORMATION AND THEREFORE CANNOT PREVENT SUCH INTERCEPTIONS OR COMPROMISES TO YOUR INFORMATION WHILE IN TRANSIT TO THE PRACTICE. ALTHOUGH WE BELIEVE THE DEVELOPER OF IMH (PRIMETIME MEDICAL SOFTWARE, WHICH COMPANY IS ALSO HEREBY INCLUDED IN THE USE OF THE TERM “IMH” OR “INSTANT MEDICAL HISTORY”) HAS ADEQUATE SECURITY MEASURES, ITS SOFTWARE, HARDWARE, AND OTHER ASPECTS OF ITS SYSTEM ARE NOT UNDER THE PRACTICE’S DIRECT CONTROL, AND THEREFORE WE CANNOT MAKE ANY WARRANTIES OR GUARANTEES AS TO THEIR SECURITY. WHEN YOU COME FOR YOUR CONSULTATION AND YOUR ELECTRONIC MEDICAL RECORD (EMR) IS CREATED, YOUR INFORMATION WILL BE IMPORTED INTO THE EMR AND IS THEN SUBJECT TO HIPAA. IF YOU ENTER INTO IMH INFORMATION FOR THE PRACTICE’S USE, BUT THEN DO NOT COME FOR INITIAL CONSULTATION AND THEREFORE DO NOT BECOME A PATIENT, WE WILL REGULARLY DELETE INFORMATION SUCH AS YOURS FROM OUR SERVER AFTER ABOUT 4 - 6 (FOUR TO SIX) WEEKS.
Your privacy is important to us, and we believe that both the Practice and IMH have instituted appropriate security measures. IMH is used by many other medical organizations, and we selected it because programs such as IMH have been shown to improve the information obtained, which we believe will lead to better medical care for you than without the use of IMH.
